Medicare
For those with Medicare, you must meet deductible and co-insurance obligations before Medicare reimburses us. We request a $223.00 deposit toward your deductible and co-insurance. Any unused remainder is refunded at the end of the year.
Commercial
We accept many commercial plans, including Regence/Blue Cross Blue Shield, Cigna, Aetna, MODA Health, Health Net, United Health, Providence Health Plan and many more. Please call our office for more information about your plan.
Uninsured
For those without insurance, no worries. We are ready to help. We charge a $100.00 fee for an office visit, provided that it is not related to chronic pain. Please call us for more information.
